Appointments ?
Although each patient is registered
with a particular doctor the patient is free to make an appointment with the doctor
of their choice. Because your notes are kept either at Goldsworth Park or Sunny
Meed Surgery we would ask that you make an appointment at the surgery where your
notes are kept.
To make an appointment at Sunny
Meed Surgery please ring 01483 766699.
To make an appointment at Goldsworth
Park please ring 01483 727136.
Routine appointments may be made
in advance (maximum of two weeks) which will enable us to offer you an appointment
suitable to your requirements.
If you cannot keep an appointment
please inform us as soon as possible.
HOW
DO I... Obtain
Home Visits ?
The practice offers home visits.
However, we would ask all patients to remember that it takes much longer to visit
patients at home and if at all possible we would appreciate it if you could attend
the surgery. Usually you will be seen more quickly than if you were visited at
home.

DO I... Obtain
Repeat Prescriptions ?
Repeat prescriptions will be issued
at the doctor's discretion and are normally for patients on long term treatments.
Requests for prescriptions can only be made in writing or faxed to us. You will
need to allow two working days for this to be processed. We are also able to send
repeat prescriptions by post if you send a stamped self-addressed envelope with
your request.
Exemption From Prescription
Charges
Those normally exempt from charges
are the under 16’s, students under 19 and in full-time education, the over
60’s, pregnant mothers and those who have recently had a baby and anyone
receiving certain benefits.
Season Tickets
Sometimes it is worthwhile buying
a “season ticket” for prepayment of prescription charges. Details
of current prescription charges and the cost of “season tickets” are
available from your pharmacy.

Obtain Sickness Certificates
You are only entitled to a DSS certificate
from your doctor for illness lasting more than seven days. For shorter periods
of ill health your employer may require you to complete a self-certification
form (SC2). DSS certificates are free but we will charge for all other certificates.
